Structure of the health system The US Department of Health and Human Services, which reports directly to the president, is responsible for the health of the nation. Ministries are the Public Health Service and the Health Care Financing Administration. The US ranks first in the world in medical spending. The United States leads the world in terms of research excellence and impact. Today, most of the Nobel Prizes in medicine are awarded to people from the USA - 18 of the 25 most recent recipients have been American citizens or visiting scientists.

There is no compulsory health insurance in the USA. The cost of health care is borne by each individual. It must be remembered that insurance does not cover everything, only a limited range of medical services. As a result, a serious injury or illness can severely cripple the family budget - medical bills account for half of private bankruptcies in the US. There are two types of employer-provided insurance:Health insurance "fee for service": payment of money for services actually provided; usually the insurance company reimburses 80% of the costs, the rest being borne by the patient.
